Indications/Uses
Vancin-S 125 mg: Antibiotic-associated pseudomembranous colitis caused by Clostridium difficile. Enterocolitis caused by Staph aureus. Vancin-S: Severe staphylococcal (including MRSA) infections eg, endocarditis, bone infection, septicemia, lower resp tract & skin infections. Severe staphylococcal infection in penicillin-allergic patient.
Dosage/Direction for Use
Vancin-S 125 mg Adult 500 mg to 2 g daily in 3 or 4 divided doses for 7-10 days. Ped 40 mg/kg daily in 3 or 4 divided doses for 7-10 days. Max: 2 g daily. Vancin-S Severe infection caused by Staph spp including MRSA & other vancomycin-susceptible organism Adult 500 mg every 6 hr or 1 g every 12 hr IV over 60 min. Childn 10 mg/kg every 6 hr or 20 mg/kg every 12 hr over 60 min. Neonate/infant Initially 15 mg/kg followed by 10 mg/kg every 12 hr & every 12 hr for infant 1 wk to 1 mth. Infection in neutropenic patients 20 mg/kg every 8 hr.
Administration
May be taken with or without food. Cap: Swallow whole, do not open. Take w/ plenty of water.
Special Precautions
Vancin-S 125 mg: W/draw use immediately if signs & symptoms suggestive of severe cutaneous adverse reactions (SCARs) including SJS, TEN, DRESS & acute generalized exanthematous pustulosis (AGEP) appear. Not to be restarted if patient developed SCAR. Not effective for other types of infections. Superinfections. Patients w/ inflammatory disorders of intestinal mucosa or Clostridium difficile-induced pseudomembranous colitis w/ concomitant renal impairment. Monitor serum vancomycin conc in patients w/ inflammatory disorders of intestinal mucosa; for skin reactions. Perform serial monitoring of renal function in patients w/ underlying renal dysfunction or those receiving concomitant therapy w/ aminoglycosides or other nephrotoxic drugs; serial tests of auditory function in patients w/ underlying hearing loss, or receiving concomitant therapy w/ ototoxic agent eg, aminoglycoside. Avoid anti-motility agents. Concomitant use w/ PPI. Pregnancy (1st trimester); may be harmful to fetal auditory nerve. Lactation. Not for childn <12 yr & adolescents unable to swallow cap. Elderly (nephrotoxicity). Vancin-S: Necrosis or severe pain in IM administration. Impaired auditory function. Renal impairment. Pregnancy & lactation. Infant & elderly especially in combination w/ aminoglycosides.
Adverse Reactions
Neutropenia, renal failure, hearing loss/ototoxicity, redneck or red man syndrome. Vancin-S 125 mg: Peripheral edema, vasculitis; fatigue, headache; abdominal pain, diarrhea, flatulence, nausea, vomiting; UTI; back pain; rashes including exfoliative dermatitis, SCARs including SJS, TEN, DRESS & AGEP; increased serum creatinine, interstitial nephritis, renal insufficiency; tinnitus, dizziness, vertigo; agranulocytosis, eosinophilia, thrombocytopenia; fever, hypokalemia. Vancin-S: Anaphylaxis. Inflammation at inj site following rapid administration.
Drug Interactions
Vancin-S 125 mg: Possible additive toxicities w/ other ototoxic &/or nephrotoxic drugs eg, aminoglycosides, amphotericin B, bacitracin, cisplatin, colistin, polymyxin B, ciclosporin, & loop diuretics. Diminished therapeutic effect w/ bile acid sequestrants. Increased serum conc w/ NSAIDs. Enhanced nephrotoxic effect of colistimethate. Enhanced nephrotoxic effect w/ piperacillin. Diminished therapeutic effect of Na picosulfate; BCG (intravesical & immunization); typhoid vaccine (live attenuated Ty21a strain). Enhanced neuromuscular-blocking effect of neuromuscular-blocking agents. Vancin-S: Concomitant use w/ anesth. Increased nephrotoxicity w/ aminoglycoside & other neurotoxic/nephrotoxic drugs.
